èíäèâèäóàëüíûõ êàðò äëÿ öåíòèëüíîé îöåíêè ôèçè÷åñêîãî è ïîëîâîãî ðàçâèòèÿ äåòåé Áåëàðóñè. Òàáëèöû è
èíäèâèäóàëüíûå êàðòû ðàçðàáîòàíû íà îñíîâàíèè äàííûõ, ïîëó÷åííûõ êàôåäðîé ïåäèàòðèè ëå÷åáíîãî ôàêóëüòåòà
Ãðîäíåíñêîãî ãîñóäàðñòâåííîãî ìåäèöèíñêîãî óíèâåðñèòåòà, è ñ óñïåõîì ïðèìåíÿþòñÿ â Ãðîäíåíñêîé äåòñêîé
îáëàñòíîé êëèíè÷åñêîé áîëüíèöå (ãëàâíûé âðà÷ Ä.Ñ. Ãóòèêîâ). Ýòà èíôîðìàöèÿ áóäåò ïîëåçíà äëÿ ïåäèàòðîâ,
ýíäîêðèíîëîãîâ è ãèãèåíèñòîâ.
Ìåòîäè÷åñêèå ðåêîìåíäàöèè óòâåðæäåíû Ìèíèñòåðñòâîì çäðàâîîõðàíåíèÿ ÐÁ â êà÷åñòâå îôèöèàëüíîãî
äîêóìåíòà
Êëþ÷åâûì ìîìåíòîì â ñîâåðøåíñòâîâàíèè äåòñêîãî çäðàâîîõðàíåíèÿ ÿâëÿåòñÿ îðãàíèçàöèÿ ïåðâè÷íîãî
êîíòðîëÿ ñîñòîÿíèÿ çäîðîâüÿ.
Âàæíûì èíñòðóìåíòîì òàêîãî êîíòðîëÿ ÿâëÿþòñÿ ðîñòîâûå ñòàíäàðòû. Ðîñò äåòåé, êàê ïðîöåññ, ÿâëÿåòñÿ
íàèëó÷øèì èíäèêàòîðîì ñòàòóñà ïèòàíèÿ è îäíîâðåìåííî ïðîãíîçîì ñîñòîÿíèÿ çäîðîâüÿ íàñåëåíèÿ â áëèæàéøåì
áóäóùåì è áèîëîãè÷åñêîé öåííîñòè ïîïóëÿöèé âîîáùå.
Èçâåñòíî, ÷òî ðèñê âîçíèêíîâåíèÿ ðàçëè÷íûõ çàáîëåâàíèé ó äåòåé äîñòîâåðíî ñâÿçàí ñ ïîêàçàòåëÿìè
ôèçè÷åñêîãî ðàçâèòèÿ.
Ñóùåñòâåííîå âëèÿíèå íà ðàçâèòèå îðãàíèçìà îêàçûâàþò íàñëåäñòâåííîñòü è îêðóæàþùàÿ ñðåäà, îäíèì èç
ýëåìåíòîâ êîòîðîé ÿâëÿåòñÿ óðáàíèçàöèÿ, íî ñòåïåíü ýòîãî âëèÿíèÿ íåîäèíàêîâà â ðàçíûõ ðåãèîíàõ.
Ñî÷åòàííîå âëèÿíèå ìíîæåñòâà øèðîêî âàðüèðóþùèõ ôàêòîðîâ ñîçäàåò â êàæäîì ðåãèîíå ñîâåðøåííî
óíèêàëüíóþ âíåøíåñðåäîâóþ ñèòóàöèþ, à ñëåäîâàòåëüíî, ïðèâîäèò ê ïîÿâëåíèþ ó æèòåëåé êàæäîãî ðåãèîíà
óíèêàëüíîé ñèñòåìû àäàïòèâíûõ ðåàêöèé.
Îäíàêî, èñòîðè÷åñêàÿ äèíàìèêà ýêîëîãè÷åñêèõ è ñîöèàëüíî-ýêîíîìè÷åñêèõ óñëîâèé æèçíè ïðèâîäèò ê
ïîÿâëåíèþ ñåêóëÿðíîãî òðåíäà. Íàëè÷èå âåêîâîé òåíäåíöèè ñïîñîáñòâóåò óâåëè÷åíèþ ðåãèîíàëüíîé
èçìåí÷èâîñòè ïîêàçàòåëåé ôèçè÷åñêîãî è ïîëîâîãî ðàçâèòèÿ. Ñëåäóåò îòìåòèòü, ÷òî èíòåíñèâíîñòü è
íàïðàâëåííîñòü ýòîãî ïðîöåññà íå ÿâëÿåòñÿ ïîñòîÿííîé.  70–80-õ ãã. íàøåãî âåêà â ðÿäå ïîïóëÿöèé
çàðåãèñòðèðîâàíî ïðåêðàùåíèå ñåêóëÿðíîãî òðåíäà è äàæå èçìåíåíèÿ ïðîòèâîïîëîæíîãî õàðàêòåðà. Â íåêîòîðûõ
ðåãèîíàõ ýòîò ïðîöåññ âîâñå íå îïðåäåëÿåòñÿ.
Òàêèì îáðàçîì, íåäîïóñòèìî ïðîèçâîäèòü îöåíêó ôèçè÷åñêîãî è ïîëîâîãî ðàçâèòèÿ ïî ñòàíäàðòàì,
ðàññ÷èòàííûì äëÿ ýòíîãåîãðàôè÷åñêîé ãðóïïû, ê êîòîðîé îáñëåäóåìûé èíäèâèä íå ïðèíàäëåæèò. Íåîáõîäèìî
òàêæå êîíòðîëèðîâàòü ðåãèîíàëüíûé ñåêóëÿðíûé òðåíä è ïðè åãî íàëè÷èè ñâîåâðåìåííî êîððåêòèðîâàòü
ñîîòâåòñòâóþùèå íîðìàòèâíûå ïîêàçàòåëè.
Öåëüþ ïðåäëàãàåìûõ ìåòîäè÷åñêèõ ðåêîìåíäàöèé ÿâëÿåòñÿ ñîñòàâëåíèå ðåãèîíàëüíûõ íîðìàòèâíûõ
òàáëèö äëÿ îöåíêè ôèçè÷åñêîãî è ïîëîâîãî ðàçâèòèÿ äåòåé Áåëàðóñè ñ ó÷åòîì ôàêòîðà óðáàíèçàöèè.
Äëÿ ðåàëèçàöèè ýòîé öåëè áûëè ïîñòàâëåíû ñëåäóþùèå çàäà÷è:
– Ñîñòàâèòü òàáëèöû íîðìàòèâîâ ìîðôîìåòðè÷åñêèõ ïîêàçàòåëåé è ïîëîâîãî ðàçâèòèÿ äëÿ äåòåé è ïîäðîñòêîâ
Áåëàðóñè.
– Ðàçðàáîòàòü àëãîðèòìû äëÿ îöåíêè ôèçè÷åñêîãî è ïîëîâîãî ðàçâèòèÿ ñ ó÷åòîì ðåãèîíàëüíûõ îñîáåííîñòåé.
Îñíîâíûå ñòàòèñòè÷åñêèå õàðàêòåðèñòèêè 18 àíòðîïîìåòðè÷åñêèõ ïðèçíàêîâ è èõ ñîîòíîøåíèé áûëè
ïðîàíàëèçèðîâàíû ó 22922 æèòåëåé Áåëàðóñè. Âîçðàñòíî-ïîëîâàÿ ñòðóêòóðà îáñëåäîâàííûõ ïðèâåäåíà â òàáë. 1.
Ó îáñëåäîâàííûõ ïî îáùåïðèíÿòûì ìåòîäèêàì (Áóíàê Â.Â., 1941) îïðåäåëÿëè äëèíó òåëà, ðóêè è íîãè, ìàññó
òåëà, îáõâàòû ãðóäíîé êëåòêè, ïëå÷à, áåäðà è ãîëåíè, äèàìåòð íèæíèõ ýïèôèçîâ ïðåäïëå÷üÿ, ïðîäîëüíûé è
ïîïåðå÷íûé äèàìåòðû ãîëîâû, òîëùèíó êîæíî-æèðîâûõ ñêëàäîê íà ïðåäïëå÷üå, ïîä ëîïàòêîé è íà æèâîòå.

Âû÷èñëÿëè: ìàññî-ðîñòîâîé ïîêàçàòåëü (ÌÐÏ) — îòíîøåíèå ìàññû òåëà (â êèëîãðàììàõ) ê êâàäðàòó äëèíû
òåëà (â ìåòðàõ), îòíîñèòåëüíóþ äëèíó íîãè — îòíîøåíèå äëèíû íîãè ê äëèíå òåëà, âûðàæåííîå â ïðîöåíòàõ,
èíäåêñ æèðîîòëîæåíèÿ — îòíîøåíèå òîëùèíû êîæíî-æèðîâîé ñêëàäêè íà ïðåäïëå÷üå ê òîëùèíå ñêëàäêè ïîä
ëîïàòêîé â ïðîöåíòàõ, à òàêæå ãîëîâíîé óêàçàòåëü — îòíîøåíèå øèðèíû ãîëîâû ê åå äëèíå â ïðîöåíòàõ.
Óñòàíîâëåíî, ÷òî ðàñïðåäåëåíèÿ òàêèõ ïàðàìåòðîâ êàê ìàññà è äëèíà òåëà, îêðóæíîñòü ãðóäè è ìàññî-ðîñòîâîé
ïîêàçàòåëü â áîëüøèíñòâå âîçðàñòíûõ ïåðèîäîâ ñóùåñòâåííî îòëè÷àþòñÿ îò íîðìàëüíîãî. Ïîýòîìó íåêîððåêòíî
èñïîëüçîâàòü èõ ñðåäíèå è ñðåäíèå êâàäðàòè÷åñêèå îòêëîíåíèÿ â êà÷åñòâå íîðìàòèâîâ ïðè èçó÷åíèè ôèçè÷åñêîãî
ðàçâèòèÿ. Òàê êàê àñèììåòðèÿ îáû÷íî íîñèò ïðàâîñòîðîííèé õàðàêòåð, òî îöåíêè áóäóò ñèñòåìàòè÷åñêè
çàíèæàòüñÿ. Îïòèìàëüíûì ïðè îáðàáîòêå äàííûõ ñ íåíîðìàëüíûì ðàñïðåäåëåíèåì ÿâëÿåòñÿ öåíòèëüíûé ìåòîä,
ïîñêîëüêó îí ïðèëîæèì ê ðàñïðåäåëåíèÿì ëþáîé ôîðìû, è íå òðåáóåò çíàíèÿ ýòîé ôîðìû.

Ðåçóëüòàòû ñòàòèñòè÷åñêîé îáðàáîòêè âñåõ èçó÷åííûõ àíòðîïîìåòðè÷åñêèõ ïàðàìåòðîâ íàìè áûëè ñâåäåíû
â òàáëèöû. Îíè ñîäåðæàò ñðåäíèå, ñðåäíèå êâàäðàòè÷íûå îòêëîíåíèÿ, çíà÷åíèÿ 3-é, 10-é, 25-é, 50-é, 75-é, 90é è 97-é ïåðöåíòèëåé äëÿ êàæäîãî ïîêàçàòåëÿ â ãðóïïàõ, ðàçáèòûõ ïî ïîëó è âîçðàñòó (ñ òðåõìåñÿ÷íûì èíòåðâàëîì).
Ýòè òàáëèöû ïðåäëàãàþòñÿ â êà÷åñòâå íîðìàòèâíûõ äëÿ îöåíêè ôèçè÷åñêîãî ðàçâèòèÿ äåòåé Áåëàðóñè.
Êðîìå òîãî, ìû ïðåäëàãàåì èíäèâèäóàëüíûå êàðòû äëÿ öåíòèëüíîé îöåíêè ôèçè÷åñêîãî è ïîëîâîãî ðàçâèòèÿ
äåòåé è ïîäðîñòêîâ Áåëàðóñè, ðàçðàáîòàííûå íà îñíîâàíèè âûøåóêàçàííûõ òàáëèö. Ïîäîáíîãî òèïà êàðòû øèðîêî
èñïîëüçóþòñÿ â ñòðàíàõ Åâðîïû è Àìåðèêè. Èõ ïîïóëÿðíîñòü â ìèðå îáúÿñíÿåòñÿ êîìïàêòíîñòüþ è
èíôîðìàòèâíîñòüþ. Ãðàôè÷åñêîå èçîáðàæåíèå öåíòèëüíûõ õàðàêòåðèñòèê èçó÷åííûõ àíòðîïîìåòðè÷åñêèõ
ïðèçíàêîâ çíà÷èòåëüíî óïðîùàåò ïðîöåäóðó îöåíêè ôèçè÷åñêîãî ðàçâèòèÿ è äåëàåò ðåçóëüòàòû áîëåå íàãëÿäíûìè.
Ñ ïîìîùüþ ñòàòèñòè÷åñêîãî ìåòîäà «Àíàëèç âðåìåííûõ ðÿäîâ» íà îñíîâàíèè ñîáñòâåííûõ äàííûõ ìû
èçó÷èëè ïîêàçàòåëè ôèçè÷åñêîãî ðàçâèòèÿ íîâîðîæäåííûõ â èíòåðâàëå ñ 1970 ïî 1990 ã. è íå îáíàðóæèëè
ñóùåñòâåííîãî òðåíäà. Îäíàêî, ïî ñðàâíåíèþ ñ èìåþùèìèñÿ â ëèòåðàòóðå íîðìàòèâàìè ýòèõ ïîêàçàòåëåé,
ïîëó÷åííûå íàìè çíà÷åíèÿ íåñêîëüêî ñìåùåíû â ìåíüøóþ ñòîðîíó. Ýòî ñìåùåíèå ìîæíî îáúÿñíèòü, òåì, ÷òî
ìû èñïîëüçîâàëè äëÿ ðàñ÷åòà äàííûå äåòåé, êîòîðûå äîæèëè äî 2 – 15 ëåò (ñðåäíèé âîçðàñò 10 ëåò 7 ìåñ.) è
ñîõðàíèëè îòíîñèòåëüíîå ôèçè÷åñêîå è ïñèõè÷åñêîå çäîðîâüå, à íåîíàòîëîãè èñïîëüçóþò ïàðàìåòðû âñåõ
ðîäèâøèõñÿ áåç ó÷åòà êàòàìíåçà. Òî åñòü ïîëó÷åííûå íàìè ðåçóëüòàòû (òàáë. 2) áëèæå ê òîìó, ÷òî ìîæíî íàçâàòü
«îïòèìàëüíûìè õàðàêòåðèñòèêàìè».

Ó äåòåé ñòàðøèõ âîçðàñòíûõ ãðóïï ñðåäíÿÿ äëèíà òåëà óâåëè÷èâàëàñü äî íà÷àëà 70-õ ãã., îäíàêî òåìïû ïðèðîñòà
ñ 60-õ ãã. ñóùåñòâåííî ñíèçèëèñü. Â ñåðåäèíå 70-õ ãã. ïðîèçîøåë ïåðåëîì ñåêóëÿðíîé òåíäåíöèè, è ðîñò
ñòàáèëèçèðîâàëñÿ. Îñòàíîâêà ïðîöåññà àêñåëåðàöèè îòìå÷åíà ó äåòåé âñåõ âîçðàñòíûõ ãðóïï, çà èñêëþ÷åíèåì
äåâóøåê 16 è 17 ëåò, êîòîðûå çà ïîñëåäíèå 20 ëåò «âûðîñëè» áîëåå ÷åì íà 0,3 σ.
Ïî íàøåìó ïðîãíîçó (âûïîëíåííîìó ñ ïîìîùüþ ìåòîäà àâòîðåãðåññèè è ïðîèíòåãðèðîâàííîãî ñêîëüçÿùåãî
ñðåäíåãî) â ïåðèîä ñ íàñòîÿùåãî âðåìåíè äî 2004 ã. ñðåäíÿÿ äëèíà òåëà äåòåé áóäåò ñîîòâåòñòâîâàòü ïðåäëàãàåìûì
íîðìàòèâíûì çíà÷åíèÿì ñ 95% äîâåðèòåëüíûì èíòåðâàëîì, íå ïðåâûøàþùèì ïî àáñîëþòíîé âåëè÷èíå ±0,15 σ.
Íàèáîëåå âûñîêèå ïîêàçàòåëè ìàññû òåëà îïðåäåëÿëèñü ó äåòåé, ðîäèâøèõñÿ â 60-õ è ïåðâóþ ïîëîâèíó 70-õ
ãã. Çàòåì ñðåäíèé âåñ óìåíüøèëñÿ, è ñî âòîðîé ïîëîâèíû 70-õ ãã. äî 1989 ã. åãî çíà÷åíèÿ ñòàáèëèçèðîâàëèñü, òî
åñòü ñåêóëÿðíûå òðåíäû ìàññû è äëèíû òåëà ó äåòåé Áåëàðóñè ñîâïàäàþò. Íàðÿäó ñ ýòèì, ìàññî-ðîñòîâîé ïîêàçàòåëü
èìååò ÿâíóþ òåíäåíöèþ ê ñíèæåíèþ.
Ñòàíäàðòèçèðîâàííîå ñîîòíîøåíèå äëèíû íîãè è ðîñòà ñ íà÷àëà 70-õ ãã. äî 90-õ ãã. èìååò òåíäåíöèþ ê
óâåëè÷åíèþ. Ñëåäîâàòåëüíî, ó áåëîðóññêèõ äåòåé â òå÷åíèå ïîñëåäíèõ äâàäöàòè ëåò ïðîèñõîäèò óìåíüøåíèå
äëèíû òóëîâèùà è óâåëè÷åíèå äëèíû íîã.
Ñåêóëÿðíûå òðåíäû îêðóæíîñòè ãðóäíîé êëåòêè ó äåâî÷åê è ìàëü÷èêîâ ðàçëè÷íûõ âîçðàñòíûõ ãðóïï íå
ñîâïàäàþò. Îõâàò ãðóäè ó äåâî÷åê 4–7 ëåò ñ 1955–1956 ãã. î÷åíü ìåäëåííî óìåíüøàåòñÿ. Ó äåâî÷åê 8–13 ëåò ýòîò
ïîêàçàòåëü âîçðàñòàë äî 1974 ã., íî ïîñëåäíèå äâàäöàòü ëåò ïðàêòè÷åñêè íå èçìåíÿåòñÿ. Ó ìàëü÷èêîâ 4–7 ëåò è ó
þíîøåé 14–17 ëåò îêðóæíîñòü ãðóäíîé êëåòêè ïëàâíî óâåëè÷èâàåòñÿ, à äëÿ âîçðàñòíîé ãðóïïû 8–13 ëåò
íîðìàòèâû óâåëè÷èâàëèñü äî 1974–1975 ãã., è çàòåì íà÷àëè ñíèæàòüñÿ. Îäíàêî èíòåãðàëüíàÿ îöåíêà äèíàìèêè
ñòàíäàðòèçèðîâàííîé îêðóæíîñòè ãðóäíîé êëåòêè íe âûÿâèëà ÿâíîé òåíäåíöèè ê èçìåíåíèþ âåëè÷èíû ýòîãî
ïàðàìåòðà.
Ãîëîâíîé óêàçàòåëü ó ðîäèâøèõñÿ â ïåðèîä ñ 40-õ ïî êîíåö 70-õ ãã. äîñòàòî÷íî ñòàáèëüíûé, íî ñ íà÷àëà 80-õ

áûòü ïðîïóùåí â ñâÿçè ñ åãî òðóäîåìêîñòüþ. Ýòîò àëãîðèòì áûë ñîçäàí äëÿ ðåàëèçàöèè êîìïüþòåðíîé âåðñèè
ïîñòðîèòåëÿ çàêëþ÷åíèé ïî äàííûì àíòðîïîìåòðèè.
 ïðèëîæåíèÿõ 50–51 ïðèâåäåíû ìàêåòû èíäèâèäóàëüíûõ êàðò äëÿ öåíòèëüíîé îöåíêè ôèçè÷åñêîãî è
ïîëîâîãî ðàçâèòèÿ äåòåé è ïîäðîñòêîâ Áåëàðóñè (50.1 è 50.2 — äëÿ äåâî÷åê, 51.1 è 51.2 — äëÿ ìàëü÷èêîâ).
Ýòè ãðàôèêè ïå÷àòàþòñÿ íà äâóõ ñòîðîíàõ ëèñòà, îòäåëüíûå ëèñòû äëÿ ìàëü÷èêîâ è äåâî÷åê. Íà ïåðâîé ñòîðîíå
èçîáðàæàþòñÿ êðèâûå äëÿ îöåíêè ìàññû è ðîñòà, íà âòîðîé — äëÿ îöåíêè îêðóæíîñòè ãðóäíîé êëåòêè è ïîëîâîãî
ðàçâèòèÿ. Òàêóþ êàðòó çàâîäÿò íà êàæäîãî ðåáåíêà è çàïîëíÿþò ïî ìåðå åãî âçðîñëåíèÿ. Èçìåðåíèÿ ïðîâîäÿò íå
ðåæå, ÷åì îäèí ðàç â ãîä. Ïîñëå êàæäîãî èçìåðåíèÿ óêàçàííûå àíòðîïîìåòðè÷åñêèå ïîêàçàòåëè íàíîñÿò íà ãðàôèê.
Ïî ãîðèçîíòàëè îòêëàäûâàþò âîçðàñò, ïî âåðòèêàëè — âåëè÷èíó ïîêàçàòåëÿ, â òî÷êå ïåðåñå÷åíèÿ ñòàâÿò ìåòêó
(îáû÷íî êðåñòèê). Êðåñòèêè ñîåäèíÿþò ëèíèåé. Åñëè ýòà ëèíèÿ íà ãðàôèêàõ ìàññû è äëèíû òåëà ñëåäóåò
ïàðàëëåëüíî öåíòèëüíûì êðèâûì â èíòåðâàëå îò 25 äî 75 ïåðöåíòèëè — ôèçè÷åñêîå ðàçâèòèå ðåáåíêà îöåíèâàþò
êàê ñðåäíåå, ãàðìîíè÷íîå. Âàðèàíòû òðàêòîâêè èíûõ ñî÷åòàíèé öåíòèëüíûõ õàðàêòåðèñòèê âåñà è ðîñòà ïðèâåäåíû
â òàáëèöå 7.2.
Êàðòû ïîçâîëÿþò çàìåòèòü òåíäåíöèþ ê íàðóøåíèþ ñêîðîñòè ðîñòà èëè ïðèáàâêè ìàññû çàäîëãî äî òîãî, êàê
âåëè÷èíà ýòèõ ïàðàìåòðîâ ïåðåñå÷åò êðèòè÷åñêèå çíà÷åíèÿ (3-þ èëè 97-þ ïåðöåíòèëü).
Ïîìîãàþò öåíòèëüíûå ãðàôèêè ïðè äèàãíîñòèêå êîíñòèòóöèîíàëüíîé ôîðìû íàðóøåíèÿ ðîñòà. Ñóùåñòâóþò
ôîðìóëû äëÿ ðàñ÷åòà îæèäàåìîãî ðîñòà ðåáåíêà â çàâèñèìîñòè îò äëèíû òåëà ðîäèòåëåé.

âîçðàñòíîé âåðòèêàëè îòìå÷àåì äëèíó òåëà òåñòèðóåìîãî, à çàòåì èç ýòîé òî÷êè ïðîâîäèì ãîðèçîíòàëüíóþ
ëèíèþ äî ïåðåñå÷åíèÿ ñ 50-é öåíòèëüíîé êðèâîé ðîñòà. Âåðòèêàëü, îïóùåííàÿ èç òî÷êè ïåðåñå÷åíèÿ, ïîêàæåò
